It begins with a comprehensive assessment of a member’s health, social, and environmental needs. An individualized care plan is produced and serves as the road map used to meet the member’s needs through care coordination.

Efficient Care Plan Development

Once members are enrolled into the program, the ILS call center conducts a telephonic assessment, which has received official cerification from the National Committee for Quality Assurance (NCQA). ILS is able to perform the telephonic assessment in 16 different languages to help eliminate cultural and language barriers. The information obtained through the assessment as well as data received from the health plan is used by ILS’ clinical team to produce an individualized care plan. A group of highly trained nurses, social workers, and care coordinators work as a team to manage the care and services identified on a member’s care plan. Services and benefits may include health plan covered benefits as well as home and community based services. The goal of the care management team is to keep the member healthy and in the least restrictive environment possible.
